- Battle.net Account Name: The current account name (email address) you are using to login.
- NEW Battle.net Account Name: (we will need you to provide us with an e-mail address not currently registered to a Battle.net account.)
- Name: The full name that the account was created with.
- Address: The address information given when the account was created.
- Confirmation that your PC has been checked for viruses.
